On September 22, 2005, Dominga was involved in a car accident that would change her life. As a result of this car accident she had headaches, upper neck pain, pain in the shoulder and shoulder blades, and pain in the left arm and lower back. Limited cervical and lumbar range of motion 10-15%. Motorbanding and tightness throughout the cervical and lumbar paravertebral musculature. Tenderness over the biceps tendon. Normal straight leg raising. 1. Cervical neuromyofascial pain, with facet involvement, most notably at the upper levels of C3 and C4. 2. Mild left biceps tendonitis. 3. Left wrist pain, secondary to sprain or an overuse syndrome, and he does note this came on after he had started a new job requiring different kinds of activities to which he was accustomed. 4. Lumbar neuromyofascial pain, with facet involvement, most likely on the right side.
